Role OverviewPerforms comprehensive nursing assessments, integrates data, and identifies problems to develop a plan of care. Maintains annual HHRG values and refers to community resources as needed.
What You Will Do
Conducts patient assessments, develops care plans, and documents patient progress. Participates in inter-disciplinary case conferences and instructs Home Health Aides.
Why It Might Be a Fit
Requires a current RN license in Mass, minimum 1 year of nursing experience, and CPR certification. Home care and IV experience, computer skills, and BSN are preferred.
