Role OverviewPerforms comprehensive nursing assessments, integrates data, and develops plans of care for patients. Maintains HHRG values, refers to community resources, and evaluates patient needs.
What You Will Do
Initiates, develops, and implements patient plans of care, documents supervision of Home Health Aides, and notifies MDs of significant changes in patient conditions.
Why It Might Be a Fit
Requires strong nursing skills, experience with home care and IV therapy, and proficiency in computer skills. Preferred qualifications include BSN and Home care experience.
Requirements
- Current RN license in Mass.
- Successful passage of VNA pre-hire medication test
- CPR certification
- Minimum one (1) year of nursing experience
- 6 months of nursing experience with a BSN
