RN Plan Case Manager Albany

Requirements

• Conduct in-home and telephonic assessments and document findings
• Develop case management care plans and monitor patient needs
• Coordinate care with community-based providers and social support programs
• Assure access to choices, primary care provider, specialists, treatment plans, and coordination of transition between care settings
• Educate patients and provide emotional support related to identified risk factors
• Implement case management interventions and work with pharmacists and patients/caregivers to ensure medication therapy management and compliance
• Function as an effective liaison between patient, PruittHealth Premier, caregivers, and other community health providers
• Organize, secure, integrate, and modify resources necessary to accomplish case management plan goals
• Timely creation of quality case management reports documenting results of CM plan interventions
• Facilitate benefits preservation through coordination of appropriate level of care and plan compliance
• Comply with PruittHealth Premier's confidentiality policy, HIPAA requirements, and state and federal regulations
