Job Description
Memorial Community Hospital & Health System is seeking a compassionate and highly organized RN Primary Care Navigator to join their team to deliver proactive, patient-centered care coordination model that improves access, enhances patient outcomes, and supports individuals in navigating the healthcare system.

Requirements

  • Graduate of an accredited School of Nursing
  • Current RN licensure in Nebraska, Iowa, or compact state
  • Current BLS certification
  • Minimum 2 years of RN experience in a clinic or provider practice
  • Experience in care coordination, case management, home health, or behavioral health
