Role OverviewThe Transitional Care Coordinator collaborates with a patient's care team to enhance the patient experience and prepare them for the next site of care. They develop and modify a patient's post-acute care plan, identify barriers to follow-up care, and facilitate necessary referrals.
What You Will Do
The Transitional Care Coordinator completes assessments, documents comprehensive plans, and facilitates necessary referrals. They communicate with the larger team, provide updates to the medical team, and develop discharge plans consistent with individual patient needs.

Requirements
- Graduate of an accredited school of nursing
- 1+ years clinical nursing experience (acute care)
- Knowledge of Medicare, Medicaid, and commercial payer regulations
- Computer competency
- Multi-tasks and prioritizes work

- Communicates effectively with persons of various backgrounds
- Maintains clinical competency regarding health care delivery/regulatory requirements
