School-Based Care Coordinator

Job Description
The School-Based Care Coordinator will serve as a key liaison among students, families, school support staff, school-based health center providers, and community-based health and mental health professionals. The coordinator will develop and strengthen partnerships with local agencies for specialty mental health support, link to internal school supports for social, academic, and behavioral functioning, and coordinate student care among a variety of school and community services.

Requirements

  • 1 year of related experience working with youth in education or behavioral health settings, and/or experience navigating complex systems of care
  • HS diploma or equivalent
  • Fluency in English, both spoken and written
  • Basic mastery of business applications and productivity suites
  • Outstanding organizational skills and ability to manage a caseload of multiple families
  • Demonstrates resilience, awareness and self-regulation in navigating challenges, conflict and obstacles/barriers
  • Thrives in a multicultural environment and works with cross-functional teams to develop and implement strategies to enhance end-user experiences
  • Awareness of local resources and navigating local resources to address stressors, or social determinants of health
  • Comfortable interacting with children and youth and committed to creating a child-friendly environment
  • Demonstrates exceptional commitment to serving and advocating for diverse and marginalized populations
  • Successfully completes tasks in a moderately noisy environment
  • Interacts with others and performs tasks in-person and using technology, with moderate periods of stationary activity and moderate periods of increased physical activity including and not limited to sitting, standing and walking
  • Moves equipment/materials weighing up to 25 pounds
  • Travels between worksites regularly
  • Works in a variety of settings, including and not limited to Seattle area schools, LCSNW office locations, partner agency and community-based settings, community settings, outdoor settings in varying weather conditions, and health service providers
