Role OverviewScreens referrals, coordinates admission processes, and provides outreach to community referral sources. Coordinates services with providers and billing reconciliation. Participates in team meetings and presents new admits, annual assessments, and hospitalizations.
What You Will Do
Markets the area for awareness of SOURCE and PRN meetings, completes new admission paperwork, and participates in team meetings. Completes quarterly home visits, quarterly PCP visits, and variances at any given time. Coordinates with outpatient therapy centers, home health agencies, and rehab.
Why It Might Be a Fit
Must have a current State License as a Registered Nurse, RN, and a valid driver's license with automobile insurance liability. Must have a minimum of five (5) years of nursing experience, with hospice or SOURCE care preferred.
