Role OverviewThe Senior Care Manager will screen referrals, coordinate admission processes, and provide outreach to community referral sources. They will also participate in team meetings, complete paperwork, and track hospital visits.
What You Will Do
The Senior Care Manager will market their area, complete new admission paperwork, participate in team meetings, complete quarterly home visits, and track hospital/ER visits. They will also coordinate with outpatient therapy centers and complete skilled care track forms.
Why It Might Be a Fit
The ideal candidate will have a current State License as a Registered Nurse, RN, and a minimum of five years of nursing experience. Hospice or SOURCE care experience is preferred.
Requirements
- Current State License as a Registered Nurse, RN
- Valid driver's license and automobile insurance liability
- Graduate of a state-approved accredited nursing school
- Certified in Basic Cardiopulmonary Resuscitation
- Minimum of five years of nursing experience. Hospice or SOURCE care preferred
