Job Description
The Senior Case Manager will lead case management and line staff for the Interim Housing Program, maintaining high-quality services and efficiency while fostering teamwork and integration among staff.

Requirements

  • Develop effective relationships with clients using a client-centered approach
  • Provide guidance to staff on implementing interventions to reduce barriers
  • Provide crisis intervention and triage services
  • Work with clients in the DMH funded Interim Housing Program (IHP)
  • Develop expert knowledge of documentation required by the agency and the funders
  • Manage intake process and ensure that all open beds remain filled
  • Assist with preparing data for reports to funders
  • Provide individual and team supervision, support, and performance evaluations
  • Facilitate weekly case conferencing to create a supportive environment
  • Communicate effectively with management, peers, collaborators, and funders
  • Maintain client confidentiality and follow agency, state, and HIPAA laws
  • Develop and improve programming, identify and implement new systems when appropriate
  • Train staff on programmatic expectations
