Job Description
The Senior Case Manager will lead the collaborative efforts of the Contingency Management Pilot mobile team in providing support and engaging participants enrolled in the Contingency Management Pilot. The pilot project supports participants to improve housing stability, health and other targeted behaviors through the use of case management services and incentives.

Requirements

  • High School Diploma or GED or Equivalent experience serving target populations.
  • Extensive experience working with chronically homeless clients
  • Three (3) or more years of case management experience
  • Certification in substance abuse counseling.
  • Computer literacy, including knowledge of Microsoft Word, Outlook and Excel
