Role Overview

The Senior Case Manager is responsible for delivering wellness and recovery-oriented case management and support services to service recipients. They provide support directly to residents, including skill development, supportive counseling, transportation, crisis intervention, and medication monitoring. The Senior Case Manager actively supports the Residence Manager/Director in the management of the program's staff and resident treatment.

What You Will Do

Provide on-site services and support to service recipients, communicate observations and information about the service recipient to the residence manager, RN, and treatment team, and document observations, information, and services provided to the service recipient in progress notes.

This position requires a valid state driver's license with a safe driving record, a Bachelor's degree in a mental health, health, or other appropriate human services related discipline, and one year of experience. The ideal candidate will have strong communication and organizational skills, and be able to work effectively in a team environment.
