Job Description
The Senior Community Health Worker provides direct patient care, health education, and community-based support services under the supervision of a Registered Nurse. Responsibilities include conducting health screenings, providing wound care, educating clients on medication use, and assisting with transportation and mobility support.
Requirements
High school diploma or GED
Four (4) years of work experience as a Certified Nursing Assistant
Current Certified Nursing Assistant (CNA) Certificate
Valid state driver's license
Ability to interact effectively with clients, families, coworkers, and healthcare professionals
Skill in delivering group presentations and community health education
Competency in computer use for research, data entry, documentation, and record maintenance
Ability to independently initiate emergency care techniques in urgent or life-threatening situations
Strong verbal communication skills and ability to follow oral and written instructions
