Job Description
The Senior Community Health Worker provides direct patient care, health education, and community-based support services under the supervision of a Registered Nurse. The incumbent conducts health screenings, provides wound care, educates clients on medication use, and assists with prenatal and postpartum care.
Requirements
High school diploma or GED
Four (4) years of work experience as a Certified Nursing Assistant
Two (2) years of experience as a Community Health Worker with the Navajo Nation CHR/Outreach Program
