Role OverviewThe Home Health RN Case Manager is responsible for planning, organizing, and directing Lifespark services while ensuring compliance with regulatory requirements and best practices. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
What You Will Do
Completes an initial assessment of client and family to determine home care needs, provides a complete physical assessment, and initiates the plan of care. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
Why It Might Be a Fit
The ideal candidate will have 2 years of RN experience in healthcare, preferably home care or hospice, and a basic understanding of OASIS. They will also have experience in customer service, supervising caregivers, and knowledge of Medicare Home Health & Hospice regulations.
Requirements
- 2 years' RN experience in healthcare (preferably home care or hospice)
- Medicare Skilled Nursing experience and basic understanding of OASIS
- Experience in customer service/direct member contact field
- Experience supervising caregivers
- Knowledge of Medicare Home Health & Hospice regulations
- Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice
- Graduate of an accredited school of nursing
- Current Minnesota Nursing License
- Must pass a MVR check based on Lifespark company policy
