Job Description
Service Coordinator I assists with daily intake and referral processes, coordinates in-home care needs, and promotes cohesiveness in the office environment. Collaborates with stakeholders regarding the delivery of services, effectively written and verbal communicator, and demonstrates dependability and problem-solving skills. Represents the company at stakeholder meetings, health fairs, and provider fairs.

Requirements

  • Bachelor’s degree in the field of Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or Related Field preferred
  • 1 year of previous experience working with individuals with disabilities or the elderly preferred
  • Combination of education and experience
  • Bilingual preferred in various locations
