Serve as the primary contact and coordinator for individuals receiving SSA services, facilitate person-centered planning conversations, and coordinate services across various systems.

Requirements
- Bachelor's degree in Human Services or a closely related field
- Eligible for DODD SSA certification
- Valid Ohio driver's license
- Acceptable driving record and insurability
- Successful background check and negative drug screen
- Proficiency with Microsoft Office, electronic case management systems, databases, virtual meeting platforms, and electronic documentation tools
