Role Overview

The Social Service Clinician Per Diem provides psychosocial and discharge planning assessments to patients and families, developing treatment plans and goals. They engage with patients and families in a direct and empathic manner, adapting approaches according to age-specific sensory, cognitive, and social abilities/needs.

What You Will Do

The Social Service Clinician Per Diem will gather, assess, and interpret psychosocial data, demonstrating awareness of age-specific factors such as signs of abuse, educational needs, functional deficits, etc. They will complete discharge planning and psychosocial assessments, identifying the patient's current and past life crisis and probable discharge plan needs.

Requirements

  • MSW or master's degree in counseling
  • Training and clinical experience in behavioral health required and necessary case management experience
  • Bachelor's degree in social work or other behavioral health specialty
  • Training and clinical experience in behavioral health required and necessary case manager experience
