Role OverviewWe rely on and trust our Part Time Social Services Coordinator to identify and provide for each resident's social, emotional and psychological needs, and the continuing development of the resident's full potential during his/her stay at the facility and to assist in the planning for his/her discharge.
What You Will Do
Develop a social history, social assessment and care plan, actively participate in interdisciplinary Care Plan meetings, document progress notes, assist with discharge and placement planning, and organize family groups.

Requirements
- At least two years of social work experience in a health care delivery system preferred
- Healthcare experience preferred
- Must meet any state and/or Federal requirements for the position
- Basic understanding of computer technology, including email use
