Role OverviewThe Lutheran Home Skilled Nursing Community is hiring a Social Services Coordinator to support resident well-being and coordinate psychosocial services for our residents and their families. This role is part of a skilled nursing team in Arlington Heights, IL. The Social Services Coordinator will work full-time, day shift, with a rotating weekend schedule.
What You Will Do
The Social Services Coordinator will complete social service assessments, coordinate discharge planning, advocate for residents and support families, facilitate family involvement in care planning meetings, connect residents with community resources, and document social service plans and progress notes accurately.
Why It Might Be a Fit
The ideal candidate will have at least 2 years of experience in a Social Services role within a skilled nursing facility, with direct experience with discharge planning in a SNF setting. A Bachelor's degree in social work, human services, or a related field is preferred.
