Job Description
Assists the Social Services Director with day-to-day activities of social services within the building assigned. Develops and maintains social work policies and procedures, communicates with residents, family, staff, and interdisciplinary team members to coordinate care and services.

Requirements

  • Possesses and demonstrates working knowledge and ability to apply standards of professional social work practice in long term care setting.
  • Develops and maintains social work policies and procedures reflecting current professional standards and philosophy of care consistent with State and Federal regulations.
  • Communicates directly with residents, family, staff, interdisciplinary team members and Department Heads to coordinate care and services, improve organization and implementation of plans of care to maintain a high quality of care.
