Role OverviewThis role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Must reside in New Jersey. The Social Work Case Manager will be dedicated to the NJ Medicaid Housing program and will be responsible for ensuring coordination of Housing Services with all other services provided to NJ Medicaid members.
What You Will Do
Cultivate stakeholder relationships, network with other community resources, provide referrals within benefits structure, develop and implement a metric system to track baseline levels of functioning, monitor and evaluate effectiveness of social work goals, and interface with Registered Nurse Care Managers and Management.
Why It Might Be a Fit
Requires MS in Social Work and minimum of 3 years of experience in case management in a healthcare environment, or any combination of education and experience that would provide an equivalent background. Knowledge of relevant public and private housing resources and stakeholders is preferred.
