Role OverviewWe are seeking a qualified Social Worker to assist our patients in our Marietta area. This role will perform visits in homes and facilities (ALF and ILF's) in their designated service area. You must have reliable transportation as travel is required daily.
What You Will Do
Initiating Advance Care Planning for patients throughout their assigned Care Team (including facilities). Completing Daily Comprehensive Assessments on patients. Completing Daily Care Plans on patients. Documenting Chronic Care Management time daily.
