Role OverviewWe are seeking a qualified Social Worker to assist our patients in our Florence area. This role will perform visits in homes and facilities (ALF and ILF's) in their designated service area.
What You Will Do
Perform visits in homes and facilities, complete daily comprehensive assessments, care plans, and document chronic care management time. Meet daily productivity goals, coordinate discharges, and build professional relationships with referral sources.
