Role OverviewProvides direct service to clients and their families, including social work assessment, interventions, and treatment plans. Works with the interdisciplinary team to develop and implement care plans. May provide supportive counseling and crisis intervention.
What You Will Do
Identifies patients who require social work assessment and intervention, performs bio-psychosocial assessments, and develops and implements care plans. May provide supportive counseling and crisis intervention, and advocates for patients and families.
Why It Might Be a Fit
Requires a Master's degree in social work or a related discipline, and a license to practice as a social worker in Minnesota. Experience with electronic health records and strong communication skills are also required.
Requirements
- Master's degree in social work or a related discipline
- LGSW, LISW, or LICSW license
- Ability to counsel and instruct patients and families
- Ability to communicate with patients and families under sometimes stressful circumstances
- Ability to recognize and document patient progress
- Ability to work cooperatively with other members of the multidisciplinary clinical team
- Ability to interact effectively with individuals from diverse backgrounds
- Experience with electronic health record or similar software program
- Effective use of all Windows-based applications
