Job Description
Join our team as a Social Worker and be an advocate for your patients and their families. As a Social Worker, you will work with individual patients and families, serve in an advocacy role, coordinate discharge planning functions, and maintain confidentiality of professional information.

Requirements

  • Bachelor's or Master's degree in social work from an accredited program
  • Knowledge of the case management process and needs of geriatric patients, including funding resources
  • Imagination and creative ability
  • Ability to work cooperatively with other professionals
  • Organizational skills and ability to carry out responsibilities efficiently and effectively
