Role OverviewThe Tremont Rehabilitation and Skilled Health Care Center is hiring a compassionate social worker to join their social services team. The role involves planning, organizing, and directing social services to meet the needs of residents. The social worker will work with residents, families, and staff to ensure the medically related emotional and social needs of the residents are met.
What You Will Do
The main responsibilities of the social worker include coordinating resident care conferences, working with individual residents and their families, and completing comprehensive documentation in a timely manner. The social worker will also utilize community resources, support staff, and advocate for residents' personal needs.

The ideal candidate will possess a social work license in Massachusetts, a bachelor's degree in social work, and excellent communication and interpersonal skills. Requirements
- Must possess a social work license in the state of Massachusetts (LSW, LICSW, LSWA, and/or LCSW), or be in the process of obtaining one.
- Must possess, as a minimum, a bachelor's in social work from an approved school of social work.
- Must be able to read, write, speak and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.
