Role OverviewThis part-time Social Worker position is responsible for planning and administering social service programs within the facility, in conjunction with the Director of Social Services. The Social Worker assists in developing policies regarding participation in community planning for health and welfare services, plans and assists in research projects, and coordinates or assists with discharge planning and coordination of community resources.
What You Will Do
The main day-to-day responsibilities of this position include assisting with planning and implementing a comprehensive social services program, identifying medical-related social needs of residents, providing counseling and support services, and maintaining records relating to residents' social work needs and care.

Requirements
- Bachelor's degree in Social Work or a bachelor's degree in a human services field
- Any certifications required by New Jersey State regulations
- Knowledge of family assessment and interventions is preferred
