Role OverviewSaugus Rehab & Nursing Center is seeking an experienced Social Worker to join our skilled nursing team. We’re looking for someone who understands the needs of long-term care residents and families and works well with an interdisciplinary team.
What You Will Do
Support residents and families throughout their stay, complete psychosocial assessments and required documentation, participate in care planning and interdisciplinary meetings, coordinate discharge planning and community resources, address resident and family concerns, collaborate with nursing, rehabilitation, admissions, and leadership.

Requirements
- Skilled nursing (SNF) or long-term care experience required
- Strong communication and organizational skills
- LCSW or LICSW preferred
