Requirements

  • Assisting Nurse Care Managers and Chronic Disease Managers in coordinating and delivering high-quality care to patients
  • Conducting outreach to patients to address non-clinical needs to coordinate care, provide support, and connect with internal resources
  • Maintaining accurate and detailed documentation on outreach, interventions, and outcomes
