Role OverviewThis role serves as a Structural Heart Nurse Navigator, coordinating patient care from referral to implant and navigating patients through the referral and screening process. The Specialty Nurse Navigator will provide patient education and review patient records to determine initial screening tests.

Formulate custom care plans, establish nursing goals, assist patients in establishing a social support network, identify and align resources, coordinate services, and work with the healthcare team to track and review data for research projects.

Requirements
- Current California RN License
- Current American Heart Association BLS and ACLS
- Three (3) years of recent RN experience in a variety of clinical settings
- Knowledge of hemodynamics and experience with advanced mechanical circulatory support; IABP, Impella, Tandem, ECMO, and LVAD
- Bachelor's degree in nursing (BSN)
