Job Description
Pinnacle Home Care is hiring a PRN Speech Therapist to provide speech therapy services to patients in North Pinellas County, FL. The ideal candidate will have a current SLP license, graduate of an accredited speech language pathology program, and at least two years of speech therapy experience.

Requirements

  • Evaluate patients' speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.
  • Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
  • Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
  • Communicate regularly with physicians, addressing any changes in patient condition.
  • Participate in the periodic revision of care plans, updating the physician and care team on patient status and obtaining additional orders as required.
  • Educate patients and families/caregivers on speech therapy techniques and related challenges.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Document all evaluation data, treatments, and patient progress promptly.
  • Supervise home health aides every 14 days when speech therapy is the sole discipline involved.
