Role OverviewProvide evaluative, consultative, and treatment interventions to home health patients referred by a licensed physician in their place of residence.
What You Will Do
Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan, and collaborate with the interdisciplinary team to develop and manage the patient care plan.

Requirements
- valid professional license in state of practice
- graduate of ASHA accredited school
- prior Home Health experience is preferable
