Role OverviewAs a SSVF Stabilization Case Manager, you will work with the Supportive Services for Veteran Families (SSVF) Team to provide case management, outreach, and community referrals to Veteran families experiencing or at risk of homelessness. You will develop relationships with landlords and property managers, and work to ensure successful outcomes and program objectives for Veteran families.
What You Will Do
You will rapidly enroll SSVF participants, ensure they qualify for the program, and connect them with community resources and stabilization services. You will also develop and maintain relationships with landlords and property managers, and work with participants to assess housing barriers, needs, and preferences.

Requirements
- Bachelor’s degree in human services
- 1 to 3 years of experience in case management, navigating community-based resources, and subsidized housing placements
- Experience working with persons experiencing or at risk for homelessness, mental health and substance use disorders, and outreach experience
- Understanding of and commitment to Motivational Interviewing, Harm Reduction, Cultural Competency, and Trauma Informed Care
- Excellent customer service skills and ability to communicate professionally with employees and enrollees
- Proficiency with electronic databases and computers
- Flexible self-starter with strong analytical skills
