Role OverviewThe Staff Nurse (RN) Home Health provides and coordinates nursing care to patients in their homes, working within the Hennepin Healthcare System's Relationship-Based Care practice model.
What You Will Do
Responsibilities include triaging patients, alleviating suffering, documenting interventions, educating patients on their disease process, and ensuring continuity of care.

Requirements
- At least 2 years of professional nursing (RN) home visiting/public health work experience

- Graduation from an accredited school of nursing
- Valid license as a Registered Nurse issued by the State of Minnesota
