Job Description
The Connecticut Department of Correction is seeking a State Program Manager to lead continuity of care activities associated with the Consolidated Appropriations Act (CAA) of 2023. The position will oversee documentation, auditing, and compliance processes, and coordinate with community health partners to support timely linkage to medical, behavioral health, pharmacy, and insurance services upon release.

Requirements

  • Manage the DOC's CAA Continuity of Care and reentry coordination program across multiple correctional facilities
  • Oversee documentation, reporting, and compliance activities related to CAA reentry provisions
