Job Description
Street Outreach Advisors work directly in the community to connect individuals with care and support services that improve health and stability. They identify and prioritize community-based organizations, build and maintain referral-generating partnerships, and facilitate warm referrals or enrollments through direct engagement.

Requirements

  • Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work
  • Comfort working independently in a fast-paced, community-facing environment
  • Ability to manage outreach activity, relationships, and follow-through across multiple partners
