Job Description
Street Outreach Advisors work directly in the community to connect individuals with care and support services that improve health and stability through in-person outreach and partnerships with community-based organizations.

Requirements

  • Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work.
  • Ability to work independently in a fast-paced, community-facing environment.
  • Valid driver's license and ability to travel locally.
  • Willingness to work occasional evenings or weekends as needed.
