Role OverviewAs a Street Outreach Case Manager, you will connect with and motivate people living on the streets to strive toward self-sufficiency, assist them in navigating the social service system, and establish relationships with community stakeholders.

Your top responsibilities will include setting goals and developing individualized case plans, ensuring timely and accurate client information entry, and leading or participating in devotions and organization activities.

Requirements
- Bachelor's degree in Social Work, Sociology, Psychology, Counseling, or another social service field
- 4 years of experience in case management, social work, or a similar setting
- Certified in Peer Support (preferred)
- Current valid Arizona driver's license with a clean driving record of 2+ years
- Excellent communication skills
- Proficient in Microsoft Office apps
- Solid and credible Christian witness and lifestyle
