Job Description
The Stroke Navigator assists the Stroke Medical Director and Program Director in improving the quality of ischemic and hemorrhagic stroke care. They contribute to performance improvement initiatives, lead interdisciplinary collaboration, and leverage Lean methodology to streamline care and support patient education. This role focuses on clinical practice, communication, and quality improvement within the stroke service line.

Requirements

  • Clinical practice and care management (a. Conducting patient rounds, assessing patient encounters, identifying barriers, supporting Joint Commission certification surveys)
  • Communication and collaboration (b. Serving as a resource to staff, collaborating with interdisciplinary teams, and facilitating care support)
  • Quality improvement (a. Issue identification, issue analysis, loop closure, and follow-up)
  • Training and education (a. Counseling staff, identifying improvement areas, and instructing on standards)
  • Other duties as assigned
