Job Description
Full Time SUBSTANCE ABUSE COUNSELOR position available in MEADVILLE, PA. Maintains communication with chemical dependency treatment team, counsels patients, and conducts group therapy.
Requirements
Maintains communication with other members of the chemical dependency treatment team.
Acts as Case Manager for patients, functioning as a social worker when necessary to prepare patients for discharge.
Counsels patients on an individual basis.
Conducts group therapy as primary facilitator.
Prepares Master Treatment Plans, Psychosocial interview forms and meaningful Aftercare Plans in concert with the needs of the patient.
