Role Overview

The SUD/DV Case Manager will support clients on their journey to recovery by connecting them with resources, coordinating referrals, and ensuring a seamless intake process. This role is essential in ensuring a high-quality, compassionate care to support individuals on their journey to mental health, recovery, and wellness.

What You Will Do

The main day-to-day responsibilities include guiding clients and families through the intake and referral process, ensuring completion of intake documentation, developing and maintaining relationships with community partners, and providing on-site case management services.

Requirements

  • Must be at least 18 years of age.
  • Social Service Worker license (SSW) or Case Management Certification (or willingness to obtain).
  • Clean criminal record (no felonies or related misdemeanors per licensing standards).
  • Ability to pass post-offer and random drug screenings.
  • Strong computer skills, including Google Workspace, data management, and email.
  • Reliable and punctual with excellent communication skills.
  • Ability to read, write, and communicate fluently in English.
