Job Description
SUDs Counselor - Homeless Outreach Team | Central City Concern's HEART Program provides proactive, street-based outreach and short-term care coordination for people experiencing homelessness.

Requirements

  • Conduct outreach in encampments, shelters, and community settings
  • Engage participants using harm reduction, motivational interviewing, and trauma-informed practices
  • Complete screenings, assessments, ASAMs when indicated, and individualized treatment plans
