Role OverviewThe Support Broker assists individuals with developing a self-directed budget and the day-to-day management of self-directed services. The role involves providing support and training to individuals and their families regarding the ongoing decisions and tasks associated with self-direction.
What You Will Do
Responsibilities include assisting the individual to develop and maintain a Circle of Support/Planning team, scheduling and facilitating planning meetings, ensuring completion of initial and annual updates to Staff Action Plans and program safeguards, and providing education and training to the individual and their planning team.

Requirements
- High School Diploma or GED
- Three years of experience in Human Services working with people with intellectual and developmental disabilities
