Job Description
Join Easterseals PORT Health's Tailored Care Management (TCM) team in Wake County to deliver whole-person, integrated care to individuals and families impacted by substance use and mental health challenges.

Requirements

  • Bachelor's degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area
  • Master's level degree preferred
  • Eligible for Qualified Mental Health Professional (QMHP) credentialing
  • Minimum 2 years of experience with mental health and/or substance use populations
  • Proficiency in Microsoft Office and Electronic Health Records (EHR)
  • Valid driver's license, current auto insurance, and a clean driving record
  • Strong communication skills and the ability to work independently
