The role involves working with recipients to determine their immediate and ongoing needs, conducting ongoing assessments, and coordinating with providers and support services to ensure comprehensive and tailored care.

Requirements

  • Bachelor's degree in a relevant field such as Psychology, Social Work, or Human Services
  • 1-2 years of experience in Behavioral Health, Substance Use, or Social Services
  • Valid NYS driver's license and access to reliable transportation
