Role OverviewThe Team Lead, Care Management will oversee a team of care managers and support staff, ensuring consistency in procedures and workflows. They will provide recommendations to improve efficiencies, monitor operational reports, and identify training needs of staff.
What You Will Do
Responsibilities include day-to-day functions, managing high acuity cases, providing oversight and coverage of the crisis queue, and coordinating case rounds. The Team Lead will also assist with orientation and training of staff, participate in the performance review process, and schedule employees.

The ideal candidate will have 3-5 years of related experience, with a minimum of 2 years direct clinical care experience as an RN. They will be able to communicate effectively with all areas of the company, be flexible and adaptable, and manage multiple projects concurrently.
Requirements
- Two to four years post-secondary schooling
- Bachelor’s degree in Nursing
- 3 to 5 years related experience
- 2 years direct clinical care experience as an RN
- Prior case management experience in a managed care setting
- Prior supervisory or team leadership experience
- Active Registered Nurse License
- State Motor Vehicle license
- CCM or other URAC approved certification within 3 years
- CT RN license must be current and unrestricted
