Job Description

As a Telehealth Health Management Nurse, you'll optimize members' level of wellness and independence by avoiding unnecessary hospitalizations or service duplication, provide continuity of care, cost containment, and improve clinical and financial outcomes.

Requirements

  • 3+ years of experience as an RN, preferably 5+ years
  • RN Diploma, Associates or Bachelors in Nursing
  • Unrestricted license in the state of residence, with multi-state/compact privileges
  • Experience using Microsoft Office, including Word, Excel and Outlook
  • Strong clinical knowledge and understanding of chronic diseases, preventive care, and health promotion
