Job Description
Evergreen Health is seeking a Testing and Education Counselor to conduct enhanced outreach and education, HIV/STI/HCV testing, and implementation of activities that address Social Determinants of Health (SDOH).

Requirements

  • Assistant with development of the annual outreach plan
  • Promotes and administers HIV/STI/HCV testing on site and at community outreach events
  • Educates and refers participants to PEP, PrEP, HIV/STI/HCV testing and treatment, care coordination, HIV and Primary Medical care
