Role OverviewTherapy Care Pharmacists utilize national and local resources to facilitate high quality care of patients with specialty pharmacy needs. They collect and assess clinical data from patients, implement and monitor care plans, communicate with prescribers to obtain orders, report outcomes, make therapeutic recommendations, and dispense prescription medications and supplies.
What You Will Do
Collect clinical data for patients, caregivers, and nurses and assess data to monitor efficacy and safety of a prescribed specialty therapies, Collaborate with prescribers to implement or modify existing treatment plans, Act as a subject matter expert and clinical knowledge resource to patients, prescribers, referral sources, and fellow team members.

Must be willing to participate in an on-call rotation, Must be able to work 100% on-site, Must have a Doctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experience, Post graduate residency and/or training Excellent clinical and customer service skills, Prior experience in home infusion and/or specialty pharmacy preferred.

- Applicable active state licensure
- Additional specialty certification or training may be required
- Prior experience with CPR+ pharmacy computer system preferred
