Role OverviewProvides information and outreach support for consumers on the MFP program, facilitating the transition of eligible individuals and performing related data entry and reporting. Establishes relationships with consumers, facility workers, and other organizations to support the transition process.
What You Will Do
Conducts initial interviews, completes intake guides, assists with transition planning, follows up with consumers, completes applications for Medicaid and other benefits, and coordinates plans with community resources.

Requirements
- High School Diploma or equivalent
- Bachelor’s Degree in Human Service or related field preferred
- At least one year of experience in a human service setting (for Bachelor’s Degree)
- At least 2 years of experience in a human service setting (for High School Diploma/equivalent)
- Thorough knowledge of community resources
- Ability to work as part of a team and communicate effectively
- Flexibility and ability to prioritize tasks
- Working knowledge of Microsoft Office products
